You have set the File Directory Path to the wrong value. It should either be blank (and therefore defaults to "images") or the name of a folder in the S3 bucket where your images are stored.
I have set it to blank and successfully uploaded an image to the bucket, in the "images" folder.

I see what you did and got an example to work by copying a mp4 and a jpg to your images folder.
However I still cannot see the other folders in my bucket (see screenshot)

I just did what you asked but I still don't see anything in my root folder in the media manager for videos.
And in the image manager I no longer see the file you put there.

Please install version 1.0.4 of the S3 Filesystem add-on - https://www.joomlacontenteditor.net/downloads/plugins/add-ons/item/amazon-s3-filesystem
In Editor Profiles -> Editor Parameters -> Filesystem, set Allow Root Access for the S3 Filesystem to Yes.

Now we see the folders, but the load time is extremely long. It takes about 10 minutes for the video files to load. Then when I open the media manager the next time its not cached. It takes another 10 minutes for the files to load.
Is there some setting to speed this up?

I am working on some optimisations, but there is not much that I can do for initial load times, as this is heavily dependant on your connection and the speed of the hosting server. Remember that the data request is sent to S3 via your server, then the data is returned via the same route to your browser.
The next update to the add-on will include some form of caching, to speed up working in folders previously visited.
