I'm using the latest version of joomla, jce editor, mediabox and img manager plugin.
I notice that the caption is shown once the first time you load the page, if you reload the page you see two-four captions as you can see here http://be.ingeniadirect.eu/puglia-recommended-hotels

The caption you are referring to is the Joomla JCaption, which is created dynamically by the JCaption script when the page loads:
This script, and all others in the <head> of your site, are being loaded 4 times.
Check that the index.php file of your template only includes:
<jdoc:include type="head" />
once.
