Even if I disable image media button from tool bar config, still appears from front-end.

Check the ordering of the editor profiles - if the profile in the screenshot is a specific "front-end" profile, make sure it is ordered before the "default" profile.

but the default profile was unpublished, so even if it's the case, my specifics profiles have to be always before default profile?

It shouldn't make any difference if it is the only published profile, but order of profiles is important, and should always be from most specific to least specific.
